As one of the largest operators of memory care communities in Virginia, we at Commonwealth Assisted Living are carefully trained to offer those experiencing memory loss, Alzheimer’s or other dementia the attention they need, as well as the safety and security they require. Sweet Memories is a resident-centered care plan with three primary components to help ensure the finest memory care available for your loved one.
Commonwealth Assisted Living has recently been awarded the Best Practice Award “One Sweet Memory” by Virginia Assisted Living Association (VALA) for going above and beyond the standards in providing the best care possible on behalf of the assisted living industry. The Sweet Memory care program is designed for those experiencing memory loss, Alzheimer’s and dementia.
VALA is, a nonprofit membership organization dedicated to providing advocacy, communication, education and resources to the assisted living industry in Virginia. VALA host events and training programs to help assisted living facilities provide the best service possible and support in managing challenges that face assisted living facility providers among its other services.
Admission
Sweet Memories helps to make the transition from home or hospital to memory care as smooth as possible. We’re with you every step of the way, navigating the move, medical care, insurance, plus the emotional toll that can accompany this difficult decision.
Evaluation Through Life History
Sweet Memories evaluation process encompasses the medical, mental, emotional, spiritual, and intellectual needs of the resident. Extensive interviews with the resident, family, and health care providers create a personalized story about his or her life.
- Who were they before their dementia?
- Where did they live?
- Were they married?
- Do they have children, grandchildren?
- What were their hobbies, career paths, pets, or travel?
Also included is a thorough evaluation of the individual’s personal habits such as sleeping or reading and his or her likes and dislikes. We evaluate cognitive, social, and mental skills to develop a complete picture of the current stage of dementia.
Resident Centered Care Plan
Once the resident evaluation is complete, our Sweet Memories experienced leadership team creates a customized care plan meeting the specific needs of each individual. We believe the big picture, combined with small nuances in each plan, make for a happier, more satisfied resident and family as each moves through the journey of dementia. Care plans are evaluated quarterly, ensuring that residents’ needs are being met each and every day.
Our warm, comfortable, and home-like environments are designed to provide the highest quality of care available, ensuring security and peace of mind as your loved one progresses through the stages of dementia. Our continuum of care is with you from the beginning stages of dementia up through and including end of life care.
Sweet Memories Care Services:
- 24-hour caregivers especially trained to assist dementia residents
- Healthy, family-style dining options that meet dining skill levels and nutritional needs
- Regularly activity schedules that stimulate the mind and body, as well as customized socialization programs designed to fit the cognitive, social and mental skills of the resident
- Open and secure outdoor spaces such as courtyards and gardens

- Quarterly resident care plan reviews
